Experiencing Problems With ChatGPT - "Error reading documents"

I am currently experiencing problems with AIPRM for ChatGpt 4.5

I keep getting the following error message when uploading documents various size documents (one page to several pages) word document and pdf.
" Error reading documents"
Is the system having problems? I have uninstalled and reinstalled the plugin with no success. I have also logout and log back in. Please advise. Thank you DJ
